Change of Address and Other Forms

Please allow up to six weeks for requested changes to take effect.

General updates:

• Update your address information.
• Submit a seasonal address.
• Change the name(s) on my mailing label.
• I'm changing churches.

Banner-specific updates:

• I'm getting two (or more) copies of the Banner.
• I should be getting the Banner but I didn't.
• I attend a Christian Reformed Church and would like to start receiving the Banner.
• I'd like to stop getting the Banner.
• I'd like to give someone a gift of the Banner.
• I don't attend a Christian Reformed Church.  Can I still get the Banner?
• Why did I get the Banner at church instead of my home?

• I have another question about my denominational record that is not answered here.
